AB  - Wearable smart monitors (WSMs) applied for the estimation of electrophysiological signals are of utmost interest for a non-stressed life. WSM which records heart muscle activities could signalize timely a life-threatening event. The heart muscle activities are typically recorded across the heart at the surface of the body; hence, a WSM monitor requires high-quality surface electrodes. The electrodes used in the clinical settings [i.e. silver/silver chloride (Ag/AgCl) with the gel] are not practical for the daily out of clinic usage. A practical WSM requires the application of a dry electrode with stable and reproducible electrical characteristics. We compared the characteristics of six types of dry electrodes and one gelled electrode during short-term recordings sessions (≈30 s) in real-life conditions: Orbital, monolithic polymer plated with Ag/AgCl, and five rectangular shaped 10 × 6 × 2 mm electrodes (Orbital, Ag electrode, Ag/AgCl electrode, gold electrode and stainless-steel AISI304). The results of a well-controlled analysis which considered motion artifacts, line noise and junction potentials suggest that among the dry electrodes Ag/AgCl performs the best. The Ag/AgCl electrode is in average three times better compared with the stainless-steel electrode often used in WSMs.

Real-time chest-wall-motion tracking by a single optical fibre grating: a prospective method for ventilator triggering

AB  - Objective: The ventilators involved in non-invasive mechanical ventilation commonly provide ventilator support via a facemask. The interface of the mask with a patient promotes air leaks that cause errors in the feedback information provided by a pneumatic sensor and hence patient-ventilator asynchrony with multiple negative consequences. Our objective is to test the possibility of using chest-wall motion measured by an optical fibre-grating sensor as a more accurate non-invasive ventilator triggering mechanism. Approach: The basic premise of our approach is that the measurement accuracy can be improved by using a triggering signal that precedes pneumatic triggering in the neuro-ventilatory coupling sequence. We propose a technique that uses the measurement of chest-wall curvature by a long-period fibre-grating sensor. The sensor was applied externally to the rib-cage and interrogated in the lateral (edge) filtering scheme. The study was performed on 34 healthy volunteers. Statistical data analysis of the time lag between the fibregrating sensor and the reference pneumotachograph was preceded by the removal of the unwanted heartbeat signal by wavelet transform processing. Main results: The results show a consistent fibregrating signal advance with respect to the standard pneumatic signal by (230 +/- 100) ms in both the inspiratory and expiratory phases. We further show that heart activity removal yields a tremendous improvement in sensor accuracy by reducing it from 60 ml to 0.3 ml. Significance: The results indicate that the proposed measurement technique may lead to a more reliable triggering decision. Its imperviousness to air leaks, non-invasiveness, low-cost and ease of implementation offer good prospects for applications in both clinical and homecare ventilation.

AB  - Accurate automated detection of premature ventricular contractions from electrocardiogram requires a training set or expert intervention. We propose a fully automated unsupervised detection method. The algorithm first clusters morphologically similar heartbeats and then performs classification based on RR intervals and morphology. Tests on clinically recorded datasets show sensitivity of 94.7%, specificity of 99.6% and accuracy of 99.5%. © 2018 IEEE.

AB  - Ejection fraction (EF) is the most used parameter for characterisation of Heart Failure (HF) condition. EF is commonly calculated using echocardiography, which is an expensive non–invasive method and not used in primary healthcare. Systolic time intervals (STI) represent a non-invasive and inexpensive method for determination of EF[1, 2]. Heart failure (HF) is the single most expensive diagnosis in medicine. 2–3% of adult population in developed countries have HF diagnosis. It is not detectable by ECG test and it is commonly detected in a late stage, when the process is irreversible [2-5]. In this paper, a multiparametric cardiac measurement system for determination of STI is presented. Measurement system consists of sensors for simultaneous acquisition of electrocardiographic (ECG), phonocardiographic (PCG), photopletysmographic (PPG) and cardiovascular (CV) pulsation signals. CV pulsation signals are measured by long period grating (LPG) fiber-optic sensors[6]. Two non-invasive methods for measuring systolic time intervals (STI) were applied on a set of 6 healthy volunteers, based on ECG, PCG and CV pulsation signals. CV pulsation signals were measured on carotide arthery with PPG and LPG sensors. In the first method, EF was calculated from the obtained STI signals, using CV carotide pulsations measured with the PPG sensor, giving EF values in the range from 0.60 to 0.68, with maximal standard deviation of 0.05. In the second method, EF was obtained using CV carotide pulsations measured with LPG sensor, giving EF values in the range from 0.60 to 0.66, with maximal standard deviation 0.06. Calculated values of EF with both methods were in the 0.55 to 0.75 range which corresponds to normal EF range in healthy individuals.

AB  - Non-invasive ventilation performed through an oronasal mask is a standard in clinical and homecare mechanical ventilation. Besides all its advantages, inevitable leaks through the mask cause errors in the feedback information provided by the airflow sensor and, hence, patient-ventilator asynchrony with multiple negative consequences. Here we investigate a new way to provide a trigger to the ventilator. The method is based on the measurement of rib cage movement at the onset of inspiration and during breathing by fibre-optic sensors. In a series of simultaneous measurements by a long-period fibre grating sensor and pneumotachograph we provide the statistical evidence of the 200 ms lag of the pneumo with respect the fibre-optic signal. The lag is registered consistently across three independent delay metrics. Further, we discuss exceptions from this trend and identify the needed improvements to the proposed fibre-sensing scheme.

AB  - In non-invasive ventilation, continuous monitoring of respiratory volumes is essential. Here, we present a method for the measurement of respiratory volumes by a single fiber-grating sensor of bending and provide the proof-of-principle by applying a calibration-test measurement procedure on a set of 18 healthy volunteers. Results establish a linear correlation between a change in lung volume and the corresponding change in a local thorax curvature. They also show good sensor accuracy in measurements of tidal and minute respiratory volumes for different types of breathing. The proposed technique does not rely on the air flow through an oronasal mask or the observation of chest movement by a clinician, which distinguishes it from the current clinical practice. AB  - Mechanical physiological pulsations are movements of a body surface incited by the movements of muscles in organs inside the body. Here we demonstrate the use of long-period grating sensors in the detection of cardio-vascular pulsations (CVP), in particular apex and carotid pulsations. To calibrate the sensors, we use a mechanical tool designed specifically to measure the sensor response to a localized perturbation at different grating curvatures as working points. From the data we infer the amplitude of the CVP. Together with the electrophysiological signals, the CVP signals obtained from the sensors can provide significant information on heart function which is inaccessible to the electrocardiogram. The low cost and easy handling of the fibre sensors increase their prospects to become the sensors of choice for novel diagnostic devices.

AB  - Respiratory-volume monitoring is an indispensable part of mechanical ventilation. Here we present a new method of the respiratory-volume measurement based on a single fibre-optical long-period sensor of bending and the correlation between torso curvature and lung volume. Unlike the commonly used air-flow based measurement methods the proposed sensor is drift-free and immune to air-leaks. In the paper, we explain the working principle of sensors, a two-step calibration-test measurement procedure and present results that establish a linear correlation between the change in the local thorax curvature and the change of the lung volume. We also discuss the advantages and limitations of these sensors with respect to the current standards.

AB  - This paper is devoted to the focusing properties of a square electrostatic rainbow lens, which is a novel ion beam optical element. We consider the transmission of parallel and non-parallel proton beams of the initial kinetic energy of 10 keV through this lens. The potential of the electrodes of the lens is chosen to be 2 kV. The electrostatic potential and components of the electric field in the region of the lens are calculated using a three-dimensional finite element computer code. We investigate the spatial and angular distributions of protons propagating through the lens and in the drift space after it. It is confirmed that the evolutions of these distributions are determined by the evolutions of the corresponding rainbow lines, generated using the theory of crystal rainbows. The beam is separated into two components. One beam component, appearing as a beam core, is generated dominantly by the focused protons. Its boundary line in the transverse position plane can be very well approximated by a hypotrochoid. The other beam component is generated dominantly by the defocused protons. We present the focusing coefficient of the lens, the confining coefficients of the lens for the focused and defocused protons, the density of the beam core, the vertical or horizontal emittance of the beam core, and the brightness of the beam core. AB  - Objective: Atrial fibrillation (AF) surveillance using a wireless handheld monitor capable of 12-lead electrocardiogram reconstruction was performed, and arrhythmia detection rate was compared with serial Holter monitoring. Methods: Twenty-five patients were monitored after an AF ablation procedure using the hand-held monitor for 2 months immediately after and then for 1 month approximately 6 months postablation. All patients underwent 12-lead 24-hour Holter monitoring at 1, 2, and 6 months postablation. Results: During months 1-2, 425 of 2942 hand-held monitor transmissions from 21 of 25 patients showed AF/atrial flutter (Afl). The frequency of detected arrhythmias decreased by month 6 to 85/1128 (P LT .01) in 15 of 23 patients. Hotter monitoring diagnosed AF/Afl in 8 of 25 and 7 of 23 patients at months 1-2 and month 6, respectively (P LT .01 compared with wireless hand-held monitor). Af/Afl diagnosis by wireless monitoring preceded Holler detection by an average of 24 days. Conclusions: Wireless monitoring with 12-lead electrocardiogram reconstruction demonstrated reliable AF/Afl detection that was more sensitive than serial 12-lead 24-hour Holter monitoring. A square electrostatic rainbow lens: Catastrophic ion beam focusing

AB  - The subject of this paper is the transmission of protons through a square electrostatic rainbow lens, which is similar to a quadrupole electrostatic lens and analogous to a crystal channel. The potential of the electrodes is chosen to be 10, 20, 50 and 100 kV. The initial proton kinetic energy is 10 keV. It is shown that the interaction potential between the proton and lens is purely anharmonic. We explore the spatial distributions of transmitted protons inside the lens and in the drift space after its exit. It is demonstrated that the evolution of the spatial distribution is determined by the evolution of the corresponding rainbow pattern. The proton beam after passing through the lens is clearly separated into the bright and dark components, which correspond to the bright and dark sides of the rainbow, respectively. All the focused protons are confined within the rainbow line. This effect is called the catastrophic ion beam focusing. We also explore the angular distributions of transmitted protons at the lens exit. The corresponding rainbow patterns appear only when the potential is 50 and 100 kV. In these cases almost all the focused protons are confined within the rainbow line. This means that the lens enables one to create a proton beam core, which includes the protons focused in both the transverse position plane and the transmission angle plane. The evolutions of the focused and defocused groups of protons are analyzed quantitatively via three parameters - the focusing coefficient of the lens, and the confining coefficients of the lens for the focused and defocused protons. AB  - Acute myocardial infarction (AMI) diagnosis in type II diabetes (DM2) patients is difficult and ECG findings are often non-diagnostic or inconclusive. We developed computer algorithms to process standard 12-lead ECG input data for quantitative 3-dimensional (3D) analysis (my3KGTM), and hypothesized that use of the my3KGTMs array of over 100 3D-based AMI diagnostic markers may improve diagnostic accuracy for AMI in DM2 patients. Methods: We identified 155 consecutive DM2 patients age GT 25 yrs with chest discomfort or shortness of breath who were evaluated at an urban emergency department (130 patients (pts)) or the cardiac catheterization laboratory (25 pts) for possible AMI. The first digital 12-lead ECG for each patient, obtained within 30 min of presentation, was evaluated by (1) 2 blinded expert cardiologists, and (2) my3KGTM. In each case, the ECG was classified as either likely AMI or likely non-AMI. Gold standard was the final clinical diagnosis. Statistical analysis was McNemars test with continuity correction. Results: The 155 DM2 patients were 50% male, mean age 56.8 +/- 12.0 yrs; 44 pts had a final clinical diagnosis of AMI (17 ST Elevation Myocardial Infarctions (STEMI), 27 Non-ST Elevation Myocardial Infarctions (NSTEMI)) and 111 had no AMI. Conclusions: Relative to standard 12L ECG read by cardiologists, quantitative 3D ECG analysis showed significant and substantial gains in sensitivity for AMI diagnosis in DM2 patients, without loss in specificity. Sensitivity gains were particularly high in patients exhibiting NSTEMI, the most common form of AMI in DM2.

AB  - Differential diagnosis of symptomatic events in post-ablation atrial fibrillation (AF) patients (pts) is important; in particular, accurate, reliable detection of AF or atrial flutter (AFL) is essential. However, existing remote monitoring devices usually require attached leads and are not suitable for prolonged monitoring; moreover, most do not provide sufficient information to assess atrial activity, since they generally monitor only 1-3 ECG leads and rely on RR interval variability for AF diagnosis. A new hand-held, wireless, symptom-activated event monitor (CardioBip; CB) does not require attached leads and hence can be conveniently used for extended periods. Moreover, CB provides data that enables remote reconstruction of full 12-lead ECG data including atrial signal information. We hypothesized that these CB features would enable accurate remote differential diagnosis of symptomatic arrhythmias in post-ablation AF pts. Methods: 21 pts who underwent catheter ablation for AF were instructed to make a CB transmission (TX) whenever palpitations, lightheadedness, or similar symptoms occurred, and at multiple times daily when asymptomatic, during a 60 day post-ablation time period. CB transmissions (TXs) were analyzed blindly by 2 expert readers, with differences adjudicated by consensus. Results: 7 pts had no symptomatic episodes during the monitoring period. 14 of 21 pts had symptomatic events and made a total of 1699 TX, 164 of which were during symptoms. TX quality was acceptable for rhythm diagnosis and atrial activity in 96%. 118 TX from 10 symptomatic pts showed AF (96 TX from 10 pts) or AFL (22 TX from 3 pts), and 46 TX from 9 pts showed frequent PACs or PVCs. No other arrhythmias were detected. Five pts made symptomatic TX during AF/AFL and also during PACs/PVCs. Conclusions: Use of CB during symptomatic episodes enabled detection and differential diagnosis of symptomatic arrhythmias. The ability of CB to provide accurate reconstruction of 12 L ECGs including atrial activity, combined with its ease of use, makes it suitable for long-term surveillance for recurrent AF in post-ablation patients.

AB  - Remote surveillance is important for patients with atrial fibrillation (AF). Atrial signal recognition with conventional monitoring devices is difficult; remote AF detection is predominantly accomplished by R-R interval analysis. Twelve lead ECG (12L) displays atrial activity and remains the gold standard for AF diagnosis. CardioBip is a portable wireless patient-activated event monitor providing signal reconstruction of a 12L waveform (12CB) using 5 leads and patient-specific transformation matrices. We hypothesized that atrial signal analysis with 12CB can detect atrial activity and improve AF detection. METHODS: 18 patients with AF undergoing DC cardioversion (CV) were studied. Separate 12-lead P and QRS patient-specific transformation matrices were created at baseline AF. Multiple wireless 12CB transmissions were performed 3-7 days before and up to 2 weeks after CV. Rhythm was confirmed with 12-lead ECGs (12L). In SR the number of leads with visible P waves (atrial signal GT 0.05 mV), and P wave polarity were analyzed. In AF, the number of leads with AF signal were compared (fibrillatory [f] waves GT 0.025 mV). RESULTS: Fourteen of 18 patients successfully cardioverted to SR and 4 failed; thus, 14 SR and 22 AF transmissions were analyzed. SR P wave was visible on 141/168 leads on 12L and 137/168 on 12CB (126 true pos [TP] and 11 false pos [FP] relative to 12L; p=0.26). In 126 leads with P waves in both 12L and 12CB, the methods agreed on P wave polarity in 125. In AF, F waves were visible in 178/264 leads on 12L and 189/264 leads on 12CB (144 TP, 45 FP; p=0.27). All 5 AF relapses were successfully detected by 12CB based on atrial activity. CONCLUSION: 12CB is not inferior to 12L in detecting atrial signal in SR and AF, and shows excellent potential for remote wireless monitoring of AF patients.

abstract = "Remote surveillance is important for patients with atrial fibrillation (AF). Atrial signal recognition with conventional monitoring devices is difficult; remote AF detection is predominantly accomplished by R-R interval analysis. Twelve lead ECG (12L) displays atrial activity and remains the gold standard for AF diagnosis. CardioBip is a portable wireless patient-activated event monitor providing signal reconstruction of a 12L waveform (12CB) using 5 leads and patient-specific transformation matrices. We hypothesized that atrial signal analysis with 12CB can detect atrial activity and improve AF detection. METHODS: 18 patients with AF undergoing DC cardioversion (CV) were studied. Separate 12-lead P and QRS patient-specific transformation matrices were created at baseline AF. Multiple wireless 12CB transmissions were performed 3-7 days before and up to 2 weeks after CV. Rhythm was confirmed with 12-lead ECGs (12L). In SR the number of leads with visible P waves (atrial signal GT 0.05 mV), and P wave polarity were analyzed. In AF, the number of leads with AF signal were compared (fibrillatory [f] waves GT 0.025 mV). RESULTS: Fourteen of 18 patients successfully cardioverted to SR and 4 failed; thus, 14 SR and 22 AF transmissions were analyzed. SR P wave was visible on 141/168 leads on 12L and 137/168 on 12CB (126 true pos [TP] and 11 false pos [FP] relative to 12L; p=0.26). In 126 leads with P waves in both 12L and 12CB, the methods agreed on P wave polarity in 125. In AF, F waves were visible in 178/264 leads on 12L and 189/264 leads on 12CB (144 TP, 45 FP; p=0.27). All 5 AF relapses were successfully detected by 12CB based on atrial activity. CONCLUSION: 12CB is not inferior to 12L in detecting atrial signal in SR and AF, and shows excellent potential for remote wireless monitoring of AF patients.",

AB  - The problem of synthesizing the standard 12-lead electrocardiogram (ECG) from the signals recorded using three special ECG leads is studied in detail. The implementation of that concept into the design of a new mobile ECG transtelephonic system is presented. The system has two separate units: a stationary diagnostic-calibration center and a mobile ECG device with integrated electrodes. The patient records by himself three special leads with the mobile ECG recorder and sends data via cellular phone to the personal computer in the diagnostic center where standard 12-lead ECG is numerically reconstructed on the base of the patient transformation matrix previously calculated into the calibration process. The experimental study shows high accuracy of the reconstructed ECG.

AB  - The VINCY Cyclotron is the main part of the TESLA Accelerator Installation. The diameter of the pole of this machine is 2000 mm. Its bending constant is 145 MeV while its focusing constant is 75 MeV. The radiofrequency system of the machine consists of two lambda/4-resonators with the eigenfrequency in the range from 17 to 31 MHz. Ions coming from a heavy ion source or a light ion source will be injected into the machine axially. They will be introduced into its median plane by a spiral inflector. Heavy ions accelerated in the machine will be extracted from it by a foil stripping system or by an electrostatic deflection system. Light ions will be extracted from it by the foil stripping system. The first programs of use of the VINCY Cyclotron are related to routine and experimental production of radioisotopes and radiopharmaceuticals, and to biomedical research with ion beams. The first beam extracted from the machine will be the proton beam of the energy of 22 MeV obtained from the H-2(+) beam extracted from the light ion source. This beam will be used first for production of radioisotope F-18 and radiopharmaceutical (18)FDG, to be employed for positron emission tomography.

AB  - A new fiber-optical dee position measurement system is developed for the VINCY cyclotron. The system includes a sensor head, a coherent optical fiber bundle and a light detector with control electronics. The sensor head is placed in the dee, all the electronic components are placed out of the vacuum chamber of the machine, while the position information is carried by the optical fiber bundle. Two sensor heads on the dee enable the measurement of the position of the dee tip in all three dimensions, as well as the twist of the dee, with a resolution better than 50 mu m. The precise dee position measurement can be useful for beam optimization, study of sparking phenomena, and study of phenomena causing errors in RF cavity geometry (vacuum, heating, etc.) These points tend to be even more important in future high current machines. Detuned primary RF coupling system for variable frequency cyclotrons

It does not require the motion of the coupling loop in vacuum and the matching for different working frequencies is performed by detuning the eigenfrequency of the amplifier cabinet alone by means of a variable capacitor. A simple mathematical approach used in the design procedure is presented. Measurements on the 1 : 1 scale model of the VINCY cyclotron RF system have been carried out in the operating frequencies range.
