Technetium-99m in Production and Use

AB  - Several types of generators have been developed for the production of (99m)Tc. Due to its excelent performances, the chromatographic type, based on the fission-produced (99)Mo sorbed in alumina, is predominant. Technetium-99m is obtained in the form of sodium pertechnetate-(99m)Tc. However, due to the known disadvantages of the production of (n, f)(99)Mo, attempts are made to avoid uranium fission. The technologies based on (n, gamma)(99)Mo (sublimation, extraction, gel) are, with the exception of the gel generator, of limited importance. Certain nuclear reactions in cyclotrons can produce (99)Mo (or directly (99m)Tc) but the obtained results are still not satisfying. Technetium-99m is used in the form of radiopharmaceuticals which are prepared by addition of (99m)Tc-eluate to the inactive components comprised in the cold kits. The chromatographic (n, f)(99)Mo/(99m)Tc generator and several (99m)Tc-radiopharmaceuticals have been developed and are regularly produced in the Vinca Institute of Nuclear Sciences (Laboratory for Radioisotopes).

Concentration of rhenium from dilute sodium chloride solutions

AB  - The conditions for the desorption of rhenium from the anion exchange resin Dowex 1-x8 by HNO3, FlCl, H2SO4 and NaOH were determined. The solution (5.0 x 10(-3) mol dm(-3) Re in 0.15 mot dm(-3) NaCl) was passed through a column containing 0.10 g of the resin. The total sorbed amount of rhenium was 0.20 g/g of the resin. It was then eluted by the corresponding eluent in the concentration range up to about 3.0 mol dm(-3). The highest elution efficiency and the most favourable elution profile were found with 3.0 mol dm(-3) HNO3. Over 77 % of the sorbed rhenium was found in the first 5 ml of the eluate. Practically all the rhenium was recovered with 20 ml of the acid. Under the given experimental conditions, HCl and H2SO4 were less favourable while NaOH was not applicable, due to very low efficiency of rhenium elution.

Separation of tungsten and rhenium on alumina in dynamic conditions

AB  - The separation of tungstate and perrhenate anions on alumina from aqueous solutions of sodium chloride was investigated in dynamic conditions. The breakthrough curves for W and Re as the function of the ratio of their concentrations in the solution, were determined. The flow rate (3 ml/min), pH 2 and NaCl concentration (0.15 mol/l) were kept constant throughout the experiments. The breakthrough capacities Q(0.05), the capacities at C/C-0 = 0.5, the total column sorption capacities Q(0.9) and the utilization degrees or column efficiencies E, were determined. According to the obtained data the separation factors alpha(W/Re) = Q(w)/Q(Re) were calculated.

Sorption of rhenium on alumina under dynamic conditions

AB  - The sorption of perrhenate anion on alumina from aqueous solutions of sodium chloride was investigated under dynamic conditions. The initial concentrations of rhenium were in the range of 2.7 x 10(-2)-2.7 mmol/dM(3). The breakthrough curves as the function of Re and NaCl concentrations (0. 12-0.20 mol/dm(3)), pH 2-6 and flow rate 3-10 ml/min were determined. According to the experimental results, it was concluded that the breakthrough capacities, the capacities at c/c(0) = 0.5, the total column capacities and the utilization degrees (column efficiency) increase with increasing Re and NaCl concentrations and decreasing pH. The optimal flow rate was found to be 3 ml/min. At pH 2 and c(0) = 2.7 mmol Re/dm(3), the total capacity was found to be Q(0.9) (max) = 2.1 x 10(-2) mmol Re/g Al2O3. The data for perrhenate anions fit with a Henry-type isotherm. It was found that, under the above mentioned conditions, the value of the Henry constant K-H was 7.8 x 10(-3) dm(3)/g and the Gibbs energy change, Delta G was -0.5 kJ/mol.

Preparation of concentrated rhenium solutions

AB  - The conditions of the desorption of rhenium from the anion exchange resin Dowex 1×8 by nitric acid were determined. The solution (5×10-3 mol/dm3  Re in 0.15 mol/dm3 NaCl) was passed through the column containing 0.1 g of the resin. The total sorbed amount of rhenium was 200 mg/g of the resin. It was then eluted by nitric acid in the concentration range of 0.16-7.2 mol/dm3 . The most favourable elution profile was found with 3.0 mol/dm3  HNO3. Over 77% of the total rhenium was desorbed with 5 ml of this eluence. Over 95% of the sorbed rhenium was recovered by using 20 ml of nitric acid in the concentration range of 0.9-7.2 mol/dm3 .

Separation of rhenium (VIII) from tungsten (VI)

AB  - Examined were the conditions for an effective separation of tungsten(VI) and rhenium(VII) on alumina if the solution of 0.20 mol dm-3 NaCl, pH=2-6, is used as the aqueous phase. Under the given experimental conditions alumina was found to be much better adsorbent for tungsten than for rhenium. The breakthrough and saturation capacities of alumina at pH = 2 are 24 and 78 mg W/g Al2O3, respectively. With the increase of pH these values decrease. So, at pH = 6 they are only 4 and 13 mg W/g Al2O3, respectively. The elution volume for rhenium for the given column dimensions and the quantity of the adsorbent, is about 16 ml. These results were confirmed by the experiments of the radiochemical separations. Tungsten-187 remains firmly bound to alumina. The radionuclidic purity of the eluted 186,188 Re at pH = 2 is very high.

AB  - The conditions for the efficient separation of tungsten(VI) and rhenium (VII) on alumina were established. The distribution coefficients K-d for tunstate and perrhenate anions, as well as the separation factors alpha (alpha = K-dWO4(2-)/K-d ReO4-) were determined using hydrochloric or nitric acid as the aqueous media. A solution of sodium chloride in the pH range 2-6 was also examined. Under all the tested experimental conditions. alumina is a much better adsorbent for tungsten than for rhenium. The obtained results indicated that the best separation of these two elements is achieved when 0.01-0.1 mol dm(-3) HCl or 1.0 mol dm(-3) HNO3 are used as the aqueous media. If NaCl is used as the aqueous phase, the best separation is achieved with 0.20 mol dm(-3) NaCl. pH 4-6. Under these experimental conditions. the breakthrough and saturation capacities of alumina for tungsten at pH 4 are 17 and 26 mg W/g Al2O3. respectively. With increasing pH, these values decrease. Thus, at pH 6 they are only 4 and 13 mg W/g Al2O3, respectively.

Chiral ligand-exchange chromatography for diastereo-enantio separation of exametazime

AB  - The diastereo-enantio separation of isomeric mixtures of exametazime (HM-PAO) by liquid chromatography is described using an achiral sorbent (RP-18). A chiral eluent with the initial complex of Cu(II) and the optically active selector N,N-dimethyl-l-phenylalanine (l-DM-PhA), based on the ligand-exchange principle, has been applied. The separation is based on the presence of the immobilized binary complex Cu(l-DM-PhA)(2) and formation of mixed ternary complex. The optimal mole ratio of Cu(II):l-DM-PhA is 1:4, the pH should be between 4.1 and 4.2 and up to 0.8 mM of triethylamine is added for column presaturation with the initial complex. The elution order has been defined using isolated l-HM-PAO via l-HM-PAO L(+)tartrate and tneso-HM-PAO obtained by repeated recrystallization from the isomeric mixture of HM-PAO. Complete resolution between all isomers (R-S from 2.14 to 3.91) and partial resolution for meso(EE)/l-HM-PAO (R-S = 0.83) has been obtained. This means that the proposed chiral ligand-exchange chromatography (CLEC) can be used for determination of the isomeric purity of HM-PAO. Radionuclidic generators for the production of technetium-99m and rhenium-188

AB  - The paper deals with production and application of open radioactive sources in modern diagnostic and therapeutic nuclear medicine. In principle, besides adequate physical, chemical and biological properties a reliable route for the routine production of both radionuclide and radiopharmaceutical should be on disposal. The favorite route are radionuclidic generators as they enable simple production of shorter-lived radionuclides at relatively low costs. The main radionuclide in diagnosis is technetium-99m. Its advantages are favorable nuclear properties, availability of large activities of high quality 99m Tc at reasonable prices by using (n,f)99Mo/ 99m Tc generators and versatile chemistry. It is used in the investigation of practically all organs and tissues in the human body. Among the beta- emitters the radionuclides of rhenium have been recently proposed as suitable candidates for therapeutic applications. The most important is 188Re which is obtained from 188W/ 188Re generator. Some of the important radiopharmaceuticals labeled with rhenium radionuclides used for certain specific indications are also shown. The activities performed in the Laboratory for Radioisotopes (Vinča Institute of Nuclear Sciences) are also enclosed.
AB  - Prikazani su neki od najvažnijih aspekata proizvodnje i primene otvorenih izvora radioaktivnog zračenja u dijagnostičkoj i terapijskoj nuklearnoj medicini. Pored fizičkih, bitne su i hemijske i biohemijske osobine datog radionuklida odnosno radiofarmaceutika. Uslov za redovnu primenu su i dostupnost, kvalitet i cena. Radionuklidni generatori predstavljaju vrlo pogodan izvor dobijanja radionuklida kraćih vremena poluraspada. Najvažniji dijagnostički radionuklid je 99m Tc. Odlične fizičke osobine, visok kvalitet i laka dostupnost po prihvatljivim cenama korišćenjem molibden-99/tehnecijum- 99m generatora i razvijena hemija doveli su do toga da ovaj radionuklid postane praktično nezamenjiv u ispitivanju skoro svih organa i tkiva u ljudskom organizmu. Dati su savremeni trendovi u razvoju proizvodnje radionuklida i radiofarmaceutika za primenu u terapijske svrhe. Prikazani su načini dobijanja radionuklida renijuma (186Re i 188Re) sa posebnim naglaskom na 188W/ 188Re generatoru. Dati su hemijski oblici i indikacije za primenu nekih radioaktivnih lekova obeleženih ovim radionuklidima. Prikazana su istraživanja sprovedena u Laboratoriji za radioizotope (Institut za nuklearne nauke «Vinča»).

Stabilization of Tc-99m radiopharmaceuticals by chemical additives

AB  - The reliability and applicability of the preparation of the three, for nuclear medicine very important, Tc-99m-radiopharmaceuticals from the inactive (technetium-cold) kit solutions were tested. Each examined commercial kit was dissolved in saline (0.9 % NaCl). The conditions of the storage of the inactive kit solutions till labeling were examined. The main problem is the stablity of the reductant stannous ions which is very difficult to predict. To stabilize and ensure a good quality of the labeled radiopharmaceuticals, ascorbic or gentisic acid were added. It was found that the best results were obtained by keeping the samples frozen at-20 degreesC. Both stabilizers can be used but for an effective protection much lower concentrations of ascorbic acid are needed. Its concentrations of 12-60 mug/ml of the kit, stabilized dimercaptosuccinate (DMS) and pyrophosphate (PyP) for about 7-8 days. The solution of 2,3-dicarboxypropane- 1,1-diphosphonate (DPD) was found to be stable even without the stabilizer. This could be attributed probably to the chemical nature of this complex. However, in routine praxis the applied procedure demands great care and personel very experienced in radiopharmacy.

Statistical evaluation of the quality control results of Tc-99m(Sn)-DPD

AB  - The results of the quality control of Tc-99m-DPD produced during five consequtive years are statistically evaluated. Radiochemical purity of the kit determined in 75 batches was 98.3+/-1.3%. TLC on silica gel with the mixture of acetone and methanole (1:1, v/v) was used to determine the content of free pertechnetate. The labeled complex and Tc-99m-hydrolyzate was separated by using ascending paper (Schleicher and Schull) chromatography and 1N NaCl as the mobile phase. Reliable results were obtained showing that the content of the impurities Tc-99m-pertechnetate and Tc-99m-hydrolyzate is 1.7+/-1.3% and 3.4+/-2.1%, respectively. The biodistribution depends on the quantity of DPD. For the animal experiments its content should be 70-80 mu g/kg b.w. The experiments revealed that the mean value of bone distribution was 8.8+/-1.9%/g and in muscles 0.043+/-0.042%/g. The uptake in liver and kidneys was below 3%, i.e., 0.65+/-0.29 and 1.71+/-0.68%/organ, respectively. The bone/muscle ratio should be at least 160. The analysis shows that the obtained values are arranged around their, statistically allowed, mean values.

abstract = "The results of the quality control of Tc-99m-DPD produced during five consequtive years are statistically evaluated. Radiochemical purity of the kit determined in 75 batches was 98.3+/-1.3%. TLC on silica gel with the mixture of acetone and methanole (1:1, v/v) was used to determine the content of free pertechnetate. The labeled complex and Tc-99m-hydrolyzate was separated by using ascending paper (Schleicher and Schull) chromatography and 1N NaCl as the mobile phase. Reliable results were obtained showing that the content of the impurities Tc-99m-pertechnetate and Tc-99m-hydrolyzate is 1.7+/-1.3% and 3.4+/-2.1%, respectively. The biodistribution depends on the quantity of DPD. For the animal experiments its content should be 70-80 mu g/kg b.w. The experiments revealed that the mean value of bone distribution was 8.8+/-1.9%/g and in muscles 0.043+/-0.042%/g. The uptake in liver and kidneys was below 3%, i.e., 0.65+/-0.29 and 1.71+/-0.68%/organ, respectively. The bone/muscle ratio should be at least 160. The analysis shows that the obtained values are arranged around their, statistically allowed, mean values.",

AB  - The in vitro stability of Tc-99m(Sn)-pyrophosphate solution was examined in dependence on the preparation conditions of the samples, the radioactive concentration of Tc-99m in the kit and the time elapsed after labeling. The samples without any protection are highly unstable. The formation of Tc-99m-pertechnetate does not depend on the radioactive concentration. Nitrogen purging provides protection in the case of low radioactive concentrations (37 MBq Tc-99m/ml) but exhibits no effect at higher concentrations. The best stabilization was achieved by using ascorbic acid. A concentration of 60 mu g/ml keeps the content of Tc-99m-pertechnetate below 1% during six hours after labeling, even in solutions of high radioactive concentrations (740-814 MBq/ml). Gentisic acid is less effective. Concentrations about ten times higher than required using ascorbic acid are needed to keep the content of Tc-99m-pertechnetate below 5% during six hours.

Stability of fractionated technetium-cold pyrophosphate solution

AB  - The cold solution of a pyrophosphate kit was prepared and dispensed into 2-ml fractions which were stored at -20 degrees C for one week. The solution was prepared by using two protection methods. The first was nitrogen purging to exclude air and the second one the addition of the well known antioxydant ascorbic acid. The aim was to determine the stability of Tc-99m(Sn)-PyP obtained by labeling these fractions prepared under the given experimental conditions. The content of Tc-99m-pertechnetate in the unprotected samples raises with time. Nitrogen purging provides some protection but already after a few days the content of pertechnetate exceeds 5%. The addition of ascorbic acid gave good results. The content of pertechnetate was 1-2% after seven days in the presence of 50 mu g of acid per mi of the cold kit.

Modified pyrophosphate-Tc-99m kit for application in nuclear cardiology

AB  - A modified Tc-99m(Sn)-pyrophosphate (PyP) kit for the application in nuclear cardiology (radioventriculography, angiocardiography, scintigraphy of blood pool) was developed. Each vial contains 12 mg PyP (Na4P2O7), 4 mg SnCl2. 2H(2)O, 2.5 mg gentisic acid and IO mg NaCl. The reconstitution is performed by dissolving the lyophilized kit in 3 mi 0.9% NaCl. In comparison with the standard pyrophosphate kit for bone scanning and detection of miocardial infarction, it contains an increased amount of Sn(Il) so that the molar ratio ligand/reductant is lowered from 25 to 2.5. The radiochemical analyses showed that the radiochemical purity of the labeled kit is high ( GT 90%) during three hours after addition of Tc-99m-activity. The shelf-life of the inactive freeze-dried preparation is up to four months providing that it is kept in vacuum and at appropriate temperature (2-8 degrees C). The biodistribution studies revealed increased accumulation in blood and low uptake by liver and kidneys. It was concluded that the modified kit performs stable and reproducible properties.

Long-lived gamma-radionuclidic impurities in spent Mo-99/Tc-99m generators

AB  - The long-lived gamma-radionuclidic impurities in the columns of spent Mo-99/Tc-99m generators one year after the calibration date have been determined by v-spectrometry. Three radionuclidic impurities fission products (Ru-103, Ru-106 and Sb-125) were detected. Also in the majority of samples, three radioisotopes of tungsten (W-181, W-185 and W-188) were also present. The contents of the impurities were found to vary,greatly. According to the activity, energy and half-life, the main contribution to the residual activity in the spent generators is due to the presence of Sb-125.

AB  - The effect of the preparation conditions on the in vitro stability of Tc-99m(Sn)-pyrophosphate kit solution has been examined. To extend the shelf-life of the preparation, different methods of protection were tested. Nitrogen purging stabilizes the kit for at least 6 h after labeling when the content of (99)mTc-pertechnetate raises to about 5%. However, this method is ineffective in the presence of hydrogen peroxide. The protecting ability of two chemicals was also determined. Gentisic acid gave good results. In the presence of 50 mu g of gentisic acid per mi of the kit the content of pertechnetate was 1-2% throughout the examined time interval. To eliminate the influence of hydrogen peroxide (6 mu g per ml of the kit) about 100 mu g of gentisic acid is needed. N,N-diphenyl-p-phenylenediamine (DPPD) performs some protecting effect only when used in the samples protected by nitrogen purging. However its protecting ability is lower that in the case of gentisic acid.
